Referral pads are usually ordered in quantities of 5 pads, but can be delivered in bulk when requested. Referral pads are sent same day via courier to minimise delivery times.

To assist with referrals, Eastern Heart Victoria accepts faxed or emailed referrals allowing easier methods of referring patients after an in-practice or telehealth consult.
We accept faxed or emailed referrals with relevant information, if you require us to contact the patient to arrange an appointment, please indicate this on the referral. You can download a fillable Eastern Heart Victoria referral form here

DICOM Images
DICOM images (for Transthoracic Echocardiograms) can be made available to referring specialists via a secure link. We do not supply viewing software however can recommend Triltech ShowCase DICOM Viewer as a suitable platform. Referrers can contact us on 8892 2222 to request patient images.
